Apply for Financing Online

Click here if you would like to apply for Chase Health Advance Financing Options Online. After providing your information our Chase will forward your informaiton to our Patient Coordinator, who will contact you to setup an appointment.

Schedule an Appointment Online

Click here if you would like to schedule a new patient examination. After providing your information our Patient Coordinator will confirm your appointment with you. Once you are scheduled please take the time to fill out any remaining forms you might need.

As part of our world-class Special Dental services, we are making new patient forms and financial policy information available to you as PDF files on this web site. You can print these forms and once complete, call to schedule an appointment and fax (858) 748-7941 these forms to our office. If you prefer, you may also complete these forms in our office prior to your first appointment.
